Alabama Permanent Makeup License Requirements

Alabama regulates permanent makeup as body art and it is up to each county’s health department to issue licenses and permits.

In order to practice permanent makeup in Alabama, you need to be associated or working with a facility that has a body art license. If you wish to go into business yourself, you will need to get this license and pass an inspection in order to practice. The license cost is $250 for initial yearly license. You will need to pay $200 every year to renew your facility license.

You will also need to apply to a body art operator permit with your county. Provide proof of Hepatitis B vaccine or declination form, a photo ID and pay the fees (determined by the county).

For example, if you lived in Jefferson county, here are the forms you’d need to fill out, as determined by the Jefferson County Department of Health.

It is illegal to tattoo anyone under 18 years of age, unless a parent or guardian signs a written consent form at the facility.

All Permits and Licenses must be renewed by Sept. 30 of each calendar year.
